Nueva Vida uses a single referral form for obstetric providers and/or OB patients. Providers can download the form, complete the requested information, and submit it by secure email, fax, or mail.

Yes. Providers can share Nueva Vida’s information with the family, direct them to the referral page, or use the single Nueva Vida Doula Referral Form when a documented referral workflow is needed.

Which referral form should providers use?

Use the Nueva Vida Doula Referral Form. It is the single form for obstetric providers and/or OB patients and can be submitted by secure email, fax, or mail.

Do doulas replace medical care?

No. Nueva Vida provides non-medical pregnancy, birth, postpartum, partner, and family support. We do not diagnose, treat, provide medical advice, or replace care from physicians, midwives, nurses, or other licensed medical providers.

Can providers call Nueva Vida for short-notice support?

Providers may contact us when a family needs support quickly, including near labor or birth. Availability, family consent, eligibility, and service fit still apply, but the team model helps us respond more consistently than a solo model.

Do families have to choose Nueva Vida?

No. Families should choose the doula support that feels right for them. Providers may share Nueva Vida alongside other available doula options.

Do you only support families with Medi-Cal?

No. Many families may qualify for Partnership HealthPlan or Medi-Cal doula support, and private-pay options are available. Eligibility and plan details apply.

Do you support Spanish-speaking families?
